Overseas Student Health Cover, or OSHC, is a mandatory requirement for most international students in Australia. It is not optional insurance; it is a visa condition, and failure to maintain adequate OSHC for the duration of your stay can result in visa cancellation. Yet many students treat OSHC as a box to tick—selecting the cheapest or most convenient policy without understanding what it covers and what it does not. OSHC is designed to help international students meet the costs of medical and hospital care while in Australia. It covers visits to doctors, some specialist services, hospital treatment, emergency ambulance services, and limited pharmaceuticals. However, OSHC is not comprehensive health insurance. It typically does not cover dental care, optical care, physiotherapy, chiropractic services, or pre-existing conditions—at least not without a waiting period or additional premium. Understanding these coverage gaps is important because a single dental emergency or a need for glasses can cost hundreds of dollars out of pocket if not covered.
The choice of OSHC provider is more than a price decision. Australia has several approved OSHC providers, including Medibank, Bupa, Allianz, NIB, and AHM. Each offers similar core coverage—because OSHC is regulated by the Australian Government—but they differ in their provider networks, direct billing arrangements, customer service, and additional benefits. A provider with a large direct-billing network means you can visit a doctor without paying upfront and claiming a rebate later. A provider with 24-hour phone support in your language may be more accessible if you need help understanding your coverage or navigating the health system.
University-preferred OSHC providers offer convenience but not necessarily the best value. Many universities have a preferred OSHC provider and offer to arrange OSHC as part of the enrolment process. This is administratively simple—the OSHC is bundled with your tuition and appears on your CoE—but you are not obliged to use the university's preferred provider. You can purchase OSHC from any approved provider, and switching from a university-arranged policy to a policy of your choice requires some administrative effort but is generally possible. Compare the university-arranged policy with alternatives before accepting the default.
OSHC timing is a visa compliance matter. You must have OSHC from the day you arrive in Australia, not just from the day your course starts. If you arrive before your course commencement, ensure your OSHC policy covers the pre-course period. Similarly, your OSHC must cover the entire duration of your student visa, including any period after your course ends if your visa allows you to remain in Australia. If your visa is extended—for example, because you change to a longer course—your OSHC must be extended accordingly. Gaps in OSHC coverage are a breach of visa conditions.
The cost of OSHC varies by provider, by the length of cover, and by whether you choose single, couple, or family cover. Single OSHC for a standard two-year master degree might cost between AUD 1,000 and AUD 2,000, depending on the provider. This cost should be included in your total study budget alongside tuition and living expenses. Some scholarships include OSHC as part of the award; if yours does, confirm the provider and coverage period to ensure it aligns with your visa duration.
Using OSHC effectively requires understanding the claims process. When you visit a doctor who direct-bills your OSHC provider, you present your OSHC membership card and pay nothing, or only a gap amount if the doctor charges above the OSHC rebate rate. When you visit a doctor who does not direct-bill, you pay the full fee upfront and then claim the rebate from your OSHC provider, typically through an online portal or mobile app. Keep your receipts for all medical visits and claim promptly—some providers have time limits for claims. If you need to see a specialist, you will generally need a referral from a general practitioner, and the specialist's fees may be higher than the OSHC rebate, leaving you with an out-of-pocket cost.
For students with ongoing health needs, consider whether OSHC is sufficient or whether supplementary private health insurance is warranted. If you have a condition that requires regular specialist visits, physiotherapy, or specific medications, OSHC's limitations may leave you with significant ongoing costs. Some OSHC providers offer extras cover as an add-on to the standard OSHC policy, covering dental, optical, and allied health services. Extras cover adds to the premium, but it may be cost-effective if you expect to use those services. Assess your health needs honestly and choose the level of cover that protects you from foreseeable costs.
Remember that OSHC is not travel insurance. It does not cover lost luggage, cancelled flights, or personal liability. If you are travelling to Australia with valuable belongings or if you want protection against travel disruptions, consider separate travel insurance for the journey and for the first few weeks of your stay. Once settled, you may decide that your OSHC and other arrangements are sufficient. The key is to understand the boundaries of each insurance policy and to ensure there are no coverage gaps that could leave you with unexpected costs.
OSHC is a small line item in the larger context of Australian study, but it protects you from the potentially very large costs of medical care in a country where healthcare is not free for international visitors.
